EMPLOYEE JOBKEEPER NOTICE OF NOMINATION FORM:

As an employee if you are eligible to receive the Jobkeeper Wage Subsidy payments, you need to download the nomination form, complete it, sign the declaration and hand it over to your employer as soon as practicable. Employers would need to confirm you have provided them with this document prior to them receiving any payments on your behalf.
